Military veteran physicians or active duty	$350 per state
FCVS Profile Setup (if applicable)	$150
Existing FCVS Profile Retrieval (if applicable)	$50 per state

Service Description:

Healthmark is committed to offering you the fastest, most reliable and thorough service in helping you obtain your medical license.  Our service includes:

·          Professional, software-driven completion of the official state medical licensing application(s)
     of your choice.

 

·          Research and collection of all appropriate mailing addresses and locations needed for credential
     verification.

 

·          Query and collection of all needed transcripts, letters of recommendation, medical malpractice
     suit history information (if applicable), letters of standing, evaluations, official exam records
     and results, medical profiles, PGT records, etc.

 

·          Regular follow-up communication with all applicable state medical boards and licensing agencies
     to ensure package completeness and prompt processing.

 

·          Regular, customized updates for you, the client, to ensure that you are informed as to every
     phase of the process.

 

·          Easy, step by step instructions informing what you, the client, must personally do by law.
     (ex. Fingerprinting, photograph submission, signatures, etc.)

 

          *   Please note, our service does not include all applicable state application fees, mailing fees,
              state filing charges, test reporting fees, and other fees the state medical boards and agencies
              charge for processing. These fees are the responsibility of the client.

          *  Though Healthmark does assist International Medical Graduates with the licensure process in the
             United States, we do not secure international credentials (ie foreign medical school diplomas,
            reference letters, etc.) from sources outside of the USA.
